What is an Anal Fissure?

An anal fissure is a tiny tear in the skin around the anus. These tears look like linear ulcers near the anal canal. They typically result in pain and bleeding, especially when you have a bowel movement. Fissures can be caused by constipation or diarrhoea. They can be short-term or long-lasting, but they often come back if the root cause isn't addressed.

Identifying the Signs of Anal Fissure

Pain during bowel movements

A noticeable tear in the anus or anal canal

Red blood during or after bowel movement

Annoying burning and itching

Understand the Causes of Anal Fissure

Anal fissures typically occur due to:

Passing hard and large stools

Straining during bowel movements because of constipation

Constant pressure on the anal tissues caused by chronic diarrhoea

Pressure on the anal tissues resulting from anal intercourse

Strain on the anal tissue during childbirth

What happens if fissures are not treated
questioning-img
  • Not treating a fissure can lead to problems like drainage, sepsis, and perforation.
  • Common injuries: Injuries, discomfort, difficulty with bowel movements, blood clots, gas, and unpredictable bowel movements.
  • Peritonitis: An intestinal fissure can lead to peritoneal inflammation and infection, called peritonitis.
  • Faecal Incontinence: Perianal fissures that are not treated or remain active can lead to faecal incontinence.
  • Abscess and fissure drainage: Fissures continue to release bad-smelling liquid, leading to worse abscesses.
  • Sepsis: This is a life-threatening situation caused by an untreated bacterial infection that can start from untreated fissures and spread throughout the body.

If you don't consult a doctor, it can result in different complications related to anal fissures, such as:

Chronic anal fissure: If an anal fissure doesn't get better within eight weeks, it's known as a chronic fissure and might need extra attention.

Recurrence of anal fissure: After having an anal fissure, the chances of it happening again are higher. If it reoccurs and affects the nearby muscles, fixing it might become more difficult. When the fissure extends into the muscle ring that keeps your anus closed (internal anal sphincter), healing could be a challenge. In such cases, medication or surgery might be necessary to relieve pain and address the fissure.

If your chronic anal fissure hasn't improved with earlier treatments or if your symptoms are severe, your doctor might suggest surgery. To relieve spasms and accelerate healing, doctors often carry out a procedure known as lateral internal sphincterotomy (LIS). This involves cutting a small part of the muscle in the anal sphincter. Research indicates that this surgery is the most successful way to treat ongoing fissures.

  • Open Fissure Surgery: Anorectal surgeons use a small incision in the anal sphincter for open surgery to loosen the anal muscle. This allows the anal fissure to heal. An ongoing concern with this widely used fissure surgery and treatment is the possibility of lasting incontinence as a side effect.

  • Laser Fissure Surgery: The patient is provided with a local or general anaesthetic before undergoing laser fissure surgery, administered by the fissure specialist.

The surgeon uses a laser probe to create infrared radiation or laser beams precisely at the surgical site, which is the location of the anal fissure. By increasing blood circulation to the fissure area, the high-energy laser beams facilitate rapid and effective healing. Sphincterotomy is a procedure where the sphincter muscle is accurately cut using a powerful laser beam. This reduces pressure on the fissure and supports the healing process of the affected area. This approach is user-friendly and promotes proper healing. Doctors commonly recommend lateral sphincterotomy as the most favoured surgery for fissures. It is the least intrusive and least painful surgical method for treating fissures.

The doctor will likely explain the entire treatment process for the fissure. Just make sure to carefully follow the instructions. If you're allergic to any medications, remember to inform the doctor beforehand.

Stop smoking and drinking at least a week before the procedure. This will help speed up and enhance your recovery and healing after the fissure surgery.

Try to avoid eating heavy meals starting the night before the fissure surgery.

  • Ensure you follow your doctor's medication instructions, applying the prescribed drug two to three times daily to the fissure surgery site or anal area.
  • Use wet wipes instead of dry tissues for a few days after bowel movements, if possible.
  • Eat plenty of fruits and vegetables.
  • Keep alcohol and caffeine intake in check to avoid disrupting bowel function and digestion.
  • Visit your fissure doctor after the procedure to stay on track for proper recovery, which greatly impacts long-term health.
  • Refrain from exercising during the first week or two post-fissure surgery and avoid heavy lifting to prevent strain on the surgical area.
  • Take sitz baths three to four times a day, especially after bowel movements, until the surgical site is fully healed.
  • It's normal to experience minor bleeding and itching at the surgical site for a few days following fissure surgery.
  • Prevent constipation by consuming a high-fibre diet, staying hydrated, and not delaying bathroom trips.

Anal fissures can lead to pain. The most intense aspect of this discomfort will occur when you have bowel movements, and it might persist for some time after that.

Lidocaine creams and similar topical pain relievers can assist with pain in specific areas, whereas ibuprofen and other anti-inflammatory pain relievers can provide relief for pain that is more spread out.

Taking regular warm showers can help the muscles of the anus relax and heal more quickly, potentially reducing certain painful symptoms.

I was advised by my physician for a circumcision for hygienic reasons as well as certain urinary problems. I found out about Mykare Health from Instagram and at first I was a bit sceptical about approaching them as I have never before availed such consultancy services, especially for a medical purpose. Anyway, with a lot of reservations in mind, I decided to take the help of Mykare Health Team. From day one including scheduling the surgery, processing the insurance related paperworks, doctor's appointments etc to the day I got discharged, as well as the follow-up medical checkups later, I have to say the whole experience was totally overwhelming. Mykare representative Ms. Greashma did a commendable job in walking me through the entire journey and, at times, she even went above and beyond her call of duty to address our concerns in a very professional manner and thankfully the entire process was smooth and hassle-free, thanks to her unwavering commitment to the task at hand. Thank you Mykare Health Team for the amazing experience. Keep this going. Kudos. 👍
